Having an in-home assessment is one of the best ways to ensure that your grandmother’s needs are being met. The person doing the assessment will ask many questions to gain as much information as possible from your grandmother and anyone else who might be there. It would be beneficial for you or someone from your family to be present at the assessment to ensure that all the information is given to the case manager. Having your grandmother’s medications handy, as well as the name of her doctor and pharmacy, will also be helpful.
